New Patients

Your First Visit

With your first visit with us at Ligocki Dental Group, we recommend that you fill out our New Patient Questionnaire online. Please remember to bring your state ID and insurance card. Once all paperwork is complete, your 90 minute new patient appointment will begin with a consultation and examination with our Doctors and our Hygiene Team to get familiar with you by reviewing your medical history and discussing your dental needs in addition to addressing any questions or concerns you may have. If it is appropriate at that time, your appointment will continue with a professional cleaning performed by a skilled dental hygienist.
