New Patient Questionnaire

With your first visit with us at Ligocki Dental Group, we recommend that you fill out the New Patient Questionnaire prior to your first visit with us. If you forget to fill out your New Patient Questionnaire online, then we recommend for you to arrive about 15-20 minutes early to allow time to fill out our new patient forms via our iPads. Please remember to bring your state ID and insurance card. Once all paperwork is complete, your appointment will begin with a consultation and examination with our Doctors and Hygienists in order to get familiar with you by reviewing your medical and health history and discussing your dental needs, in addition to addressing any questions or concerns you may have. If it is appropriate at that time, then your appointment will continue with a professional cleaning performed by our skilled clinical professional. Can't wait to meet you!
